About the role



Are you a Physiotherapist that is passionate about enabling clients with chronic disease to optimise their functional potential and quality of life whilst reducing their risk of hospital admission?

As a Grade 2 HARP Physiotherapist, you will be responsible for the provision of physiotherapy services to community dwelling patients with a variety of chronic illnesses, with the intent of reducing hospital admission risk. You will be expected to possess clinical expertise and relevant depth of experience in the area of frailty assessment and chronic disease management, particularly related (but not exclusive) to cardiac and pulmonary disease. This is a Temporary position that will be working across the multi-site Integrated services acute and aged medicine – Hospital Admission Risk Program and Chronic Rehabilitation



Integrated services Hospital Admission Risk Program is a Health Independence program guided by the Health Independence Model of care. It is a time-limited, goal-based program providing complex care coordination to patients who have chronic disease, and or complex psychosocial care needs. Workload will include the provision of both centre and community-based therapy, inclusive of providing assessments and exercise sessions within the Chronic Rehabilitation program. Care is delivered within an integrated service utilising various disciplines within the program and in partnerships with other community based programs.



About you



To be successful in this role you should possess:




  • Current recognised qualifications in Physiotherapy

  • Current registration with AHPRA 

  • Post Graduate qualifications in Physiotherapy(Desirable)

  • Previous experience working with complex rehabilitation clients

  • Highly developed interpersonal and leadership experience

  • A strong commitment to high quality care

  • The ability to work as part of a diverse team

  • Excellent communication and organisational skills

  • Current Victorian Drivers Licence
